Talking Titanium cards and box chip

Hi guys just wanted to learn more about how the cable boxes are chipped and what this involves and also if any knows a beginners sight on how to program titanium cards would be grateful.

Im currently using a titanium card now on my cable box but not getting all the channels anyone have any ideas of what this could be?

Thanks Datamine.
