what is a "patched" receiver?

Hi,new to this hobby,and not yet bought a receiver yet.A lot of people talk about a receiver being "patched" to a particular cam.What does it mean?
Will I still need to purchase a CAM if I want to view certain channels,or is the patched cam already enbedded in the receiver,so all I will need to buy is the viewing card.l
