sky + help needed

Hi all my sky plus is not working just says on screen "system fault" have tried to do a system reset to no avail.I also have forced a softwear upgrade. When i try to do a health check it says on the screen your incident number is 5208 please enter a four digit access code. Does any one know these codes ?
Thanks in advance Tony Martin
