My System: Sky+, DB 7000s, Gemini 4.3 in flash, Var on USB stick. Transparent 80cm Dish, Moteck SG2100 DiseqC motor, lots of legacy gear. Meters: Satlook Digital NIT, Unaohm EP313, Swires Annie 204 Spectrum, Rover ST-4 Spectrum.

Quite easily, they encrypt them and you require a decoder to view the channels.

My System: Mostly gathering dust cos I'm a lazy git. Porty dish face blew off the mount, not too pleased...

The Sleepy Hollow pollracle has spoken!

That's how On/ITV DIgital Used to and TopUpTV currently work, it's not that different to how we get pay TV via satellite, it's just the transmission technique's different...

ITV was launched in the Uk, way back in 1955. _http://www.zen77094.zen.co.uk/vintagebroadcasting/itvlaunch.htm

No idea about the US, but probably a little earlier, early 40s.
